About Able Victoria 关于我们

Welcome to Able Victoria — a registered NDIS provider dedicated to delivering high-quality social work services.
We specialise in Support Coordination, Specialist Support Coordination, and Psychosocial Recovery Coaching, offering tailored, person-centred support to help you work towards your goals and improve your quality of life.

Our founding member is an accredited social worker with extensive experience across a variety of settings, including clinical social work in the public mental health system, case management within the criminal justice system (corrections and courts), homelessness services, and both physical and psychosocial disability support. They also bring valuable expertise from working directly with the NDIS as a Local Area Coordinator, ensuring you receive informed guidance and support throughout your NDIS journey.

At Able Victoria, we are committed to delivering services that respect your individuality, culture, and lived experience. Our team is culturally diverse, LGBTQI+ friendly, and fluent in both English and Chinese, allowing us to provide inclusive, accessible, and compassionate support to everyone we work with.

With extensive experience in NDIS Local Area Coordination and knowledge of NDIS guidelines and legislation, we provide personalized support to help you:

Implement funded supports to meet your needs and goals.
Connect with community and mainstream services (e.g., medical, housing, employment).
Identify strengths, acquire skills, and have control over your plan.
Optimize your plan by screening providers, coordinating supports, and assisting with plan reviews.

Specialist Support Coordination - 专业支持协调

Specialist support coordination is an advanced level of support coordination, designed for individuals with more complex situations who require specialized assistance in managing and coordinating their supports and services. If you require this level of support, you will be paired with one of our senior support coordinators, who holds a tertiary qualification in Social Work and has extensive experience working with individuals with complex needs. We will ensure that you receive the tailored support necessary to achieve your goals.

The NDIS Psychosocial Recovery Coaching line item provides tailored support for individuals with mental health challenges, helping them improve independence, well-being, and life satisfaction. It includes creating personalized recovery plans, developing coping strategies, and connecting participants to mental health and community resources.

How to Provide:

Regular coaching sessions focused on strengths, goal setting, and skill development.
Collaborate with the participant, family, and other providers for holistic support.

Potential Outcomes:

Improved mental health, daily living skills, and community involvement.
Greater independence, confidence, and empowerment in managing mental health challenges.
Psychosocial Recovery Coaching aims to support long-term recovery and a fulfilling life based on individual goals.

Social Work Service - 社会工作服务

Social work services offer a range of support designed to empower individuals and families in need. These services include psychosocial assessments, Alcohol and Other Drugs (AOD) assessments, family violence risk assessments, and child protection liaison. Social workers provide family consultations, therapeutic support, psychoeducation, and advocacy for rights. They also facilitate referrals to mainstream services such as public housing, AOD treatment, and public mental health services, ensuring individuals are connected to the right resources for their unique needs. Social work services aim to support well-being, enhance coping strategies, and promote positive outcomes across various aspects of life.

At Able Victoria, we are committed to supporting you from the very beginning of your NDIS journey. We offer a free initial consultation to help establish your NDIS eligibility and provide expert guidance throughout the NDIS access and planning process, drawing on our extensive experience in NDIS Local Area Coordination (LAC).

If you are considering applying for the NDIS but are unsure of the process, we can assist you with completing the NDIS Access Request Form and provide advice on preparing the necessary supporting documents. Additionally, we can support you in preparing for your pre-planning meeting, ensuring you are fully prepared and stress-free. With our in-depth knowledge of the NDIS and understanding of what the NDIA will require, we can increase your chances of achieving the best possible outcome from your pre-planning meeting.
